Workshop on: Transvaginal evaluation of gynecological pathology

Aim of the course

In the recent years the combination of transvaginal sonography withadvanced Doppler techniques (color/power) and three-dimensional (3DUS) approaches that allow the identification of the coronal scanning plane has made transvaginal ultrasound an indispensable tool in the diagnosis of gynaecological diseases.
The introduction into clinical practice of modern high-frequency endovaginal probes has enabled gynaecologists to achieve a high diagnostic accuracy in the study of uterine (endometrial and myometrial) and ovarian pathologies.
The addition of color/power Doppler in the study of pelvic organs has greatly contributed to improving the diagnostic accuracy of ultrasound in gynaecological pathologies combining the morphological features of the detected lesion with several important data related to the functional
study of their vascular pattern.
In the diagnosis of uterine pathologies, transvaginal sonography allows a more detailed study of uterine cavity and of myometrial structure, allowing an accurate diagnosis of some lesions, such as submucous myomas, endometrial polyps and adenomyosis, characterized by very disturbing symptoms (such as menorrhagia, chronic pelvic pain, dyspareunia, dysmenorrhea).
Moreover, the high frequency endocavitary probes allowed operators to gain more detailed information about the ultrasonographic macroscopic morphology of the gonads, allowing a good correlation between morphological US features and pathophysiological ovarian conditions.
The aim of this course is to give 1) a correct methodology to perform transvaginal evaluation of the uterus and 2) a complete overview of the diagnostic ability of transvaginal ultrasound in the differential diagnosis of the most common benign uterine and ovarian lesions, in fertile age and in menopause, as submucous myomas, adenomyosis, endometrial polyps and hyperplasia, dysfunctional ovarian cysts, ovarian masses and endometriosis.
The contribution of Doppler evaluation in the diagnosis of these benign conditions is fundamental today.
The ultrasound exams will be performed on patients affected by uterine/adnexal pathology by using a tranvaginal approach implemented by color/power Doppler and 3D-US.

Objectives
Each course (lectures and practical sessions) is designed to improve practical skills and interpretation of the operators who perform ultrasound for diagnostic purposes in the field of obstetrics and gynecology, by using latest generation ultrasound equipment. At the end of each course, trainees will be able to perform advanced ultrasound examinations, be well oriented in the choice of the correct ultrasound approach to use in different clinical situations and be able to correctly understand all the different sonographic features which they will have to face in clinical practice.

Learning objectives

  • Attending the conference, participants will acquire:
  • Correct methodology to perform a good transvaginal scan of pelvic organs
  • Essential criteria for the differential diagnosis of the endocavitary uterine pathology and ovarian cysts
  • Rules to optimize the acquisition of diagnostic sonographic images
  • Updates on the diagnostic capabilities of the new transabdominal high frequencies probe (apple probe) in gynaecological lesions

Guided practical activity
The learning pathway will take place through the execution of: 1) theoretical learning sessions about interactive ultrasound, 2) practical sessions using the latest generation ultrasound equipment, technologically advanced. Each exam will be performed by a trainee under the direct supervision of a tutor who is highly experienced in ultrasound. The examinations will be performed on women in different periods of pregnancy and on gynecological patients, by endovaginal and transabdominal approaches, using B-mode, three- and fourdimensional modules, and color / power Doppler velocimetry.

US monitoring of fetal growth Siena January 2012

Aim of the course
The ultrasound scan is currently considered to be a safe, non-invasive, accurate and cost-effective investigation in the fetus. It has progressively become an indispensable obstetric tool and plays an important role in the care of every pregnant woman.
Monitoring of fetal growth is fundamental in modern perinatology, because it is strictly related to fetal/neonatal wellbeing. Moreover, the identification of abnormal intrauterine growth patterns enables better pregnancy management. Monitoring of fetal growth finds its most important investigation technique in ultrasound. Indeed, the sonographic detection of fetal biometric parameters is the basis of both an accurate gestational age assessment and a correct identification of intrauterine growth abnormalities (as fetal growth restriction and fetal macrosomia). An important clinical role is attributed today to the ultrasound estimation of fetal weight (EFW).
The aim of this course is to give a complete understanding of the actual role of ultrasound in determining fetal growth, weight and well-being. Several key aspects will be approached to better achieve a good diagnostic accuracy of fetal growth disorders (SGA fetuses, macrosomia, IUGR).
Moreover, a new method for estimating fetal weight, based on an artificial neural network, will be clinically presented and tested (Fetal Weight Index).
The ultrasound obstetric examinations will be performed on pregnant women in different periods of gestation (second and third trimester) using a transabdominal approach implemented by Doppler velocimetry (color/power) and 3D-4D modules.

Each course (lectures and practical sessions) is designed to improve the practical skills and interpretation of the operators who perform ultrasound for diagnostic purposes in the field of obstetrics and gynecology, by using latest generation ultrasound equipment. At the end of each course, trainees will be able to perform advanced ultrasound examinations, be well oriented in the choice of the correct ultrasound approach to use in different clinical situations and be able to correctly understand all the different sonographic features which they will have to face in the clinical practice. Appropriate technical background and qualified personnel are present to provide cost-effective care.
Learning objectives
Attending the conference, participants will acquire:

  • Overviews on the role of US in fetal growth disorders
  • Criteria to optimize the acquisition of fetal biometry and estimate fetal weight
  • Updates on 3D/4D ultrasound in obstetrics.

Guided practical activity
The learning pathway will take place through the execution of: 1) theoretical learning sessions about interactive ultrasound, 2) practical sessions using the latest generation of technologically advanced ultrasound equipment. Workshop on: Second Trimester normal fetal echocardiography
Aim of the course
The prenatal diagnosis of congenital heart diseases is very important today in optimizing obstetrical and neonatal care. The incidence of congenital heart diseases is around 8 per 10.000 live births, but the true incidence in the fetus is supposed to be much higher. Ultrasound techniques are the primary diagnostic and monitoring modalities in fetal cardiology. The recent revolutionary advances in ultrasound imaging technology have also allowed a most accurate study of the evolution of congenital heart disease during fetal life. Today, it is well known that, over the course of gestation, altered pressure and flow relationships may result in significant secondary damage to the developing heart, significantly increasing postnatal mortality and morbidity.
There are several indications for fetal echocardiography. The most common are represented by: 1) a positive family history, 2) maternal diabetes mellitus, 3) a suspicion arising during a routine ultrasound examination, 4) fetal arhythmia, 5) LES, 6) detection of an extracardiac congenital anomaly and 7) teratogen exposure. Although high risk populations must be referred to highly specialized centres, very often ultrasound operators are faced every day with the evaluation of normal fetal heart anatomy and today there is a great need to learn how to perform a correct basic level echocardiography.
Fetal echocardiography requires the use of high resolution ultrasound equipment, adequate transducer frequencies and specific system settings that must be changed in real time to reach the best resolution in each patient and in each condition.
Doppler techniques for example can be used to visualize blood flow through the heart, great vessels, and umbilical vessels. Knowledge of complex congenital heart disease before delivery allows therapeutic interventions to begin immediately after birth and can be life saving for such patients.
The aim of this course is to give a complete understanding of the ultrasound settings needed to perform good echocardiography.
Moreover, the most important scanning planes of the normal fetal heart (apical four chamber view, subcostal four chamber view, long-axis aorta, long-axis pulmonary artery, short-axis ventricles, aortic arch view) will be shown and explained from a clinical and diagnostic point of view  Some important rules and practical suggestions will be discussed.
The ultrasound exams will be performed on pregnant women between 20 and 26 week of gestation using a transabdominal approach implemented by color/power Doppler velocimetry.

Each course (lectures and practical sessions) is designed to improve thepractical skills and interpretation of the operators who perform ultrasound for diagnostic purposes in the field of obstetrics and gynecology, by using latest generation ultrasound equipment. At the end of each course, trainees will be able to perform advanced ultrasound examinations, be well oriented in the choice of the correct ultrasound approach to use in different clinical situations and be able to correctly understand all the different sonographic features which they will have to face in clinical practice.


Learning objectives
Attending the conference, participants will acquire:
•          Overviews on the main machine settings and scanning planes required to perform a basic evaluation of fetal heart
•          Knowledge of the normal heart anatomy in the different planes, and of the sequential approach to the fetal heart
•          Basic knowledge required to correctly interpret the vast majority of examinations
•          Ability to reach the main views of the fetal heart
•          Criteria to optimize the acquisition of the correct images
•          Updates on the role of color/power Doppler in fetal echocardiography

 

Guided practical activity
The learning pathway will take place through the execution of:
1) theoretical learning sessions about interactive ultrasound
2) practical sessions using the latest generation of technologically advanced ultrasound equipment.
